Plan Evaluation Request Form

The first step in participating in the 2007 WUI Grant process is to have your defensible space project reviewed and evaluated by the area Colorado State District Forester, CK Morey. In an effort best utilize CK's time, we have developed this on-line questionaire.

Please fill out this form on line and when done, hit the submit button to be placed on the list for evaluation. Do your homework first, and have a good plan in mind. Be brief, and succincnt in your answers. This form is designed to gather preliminary information for further evaluation. Your responses will be forwarded to CK so that he may plan a site visit to review your project.
